As the parent or legal guardian of the participant I am registering for the Kennedy Soccer Goal Keeper Camp, I hereby release and discharge, indemnify and hold harmless Denver Public Schools, John F. Kennedy High School, Stigma, Coach Matt Salis and all of his assistant coaches, against all claims, demands, and causes of action whatsoever, either in law or in equity, relating to injury, disability, death or other harm, to person or property or both, arising from my registered minor's participation in and/or presence at the Kennedy Soccer Goal Keeper Camp. I acknowledge that I am aware of the hazards and risks which may be associated with participation. I further request and authorize the personnel of Kennedy Soccer to refer to an appropriate medical facility, for treatment of illness, injury or both; and I further authorize the medical professional(s) selected by the camp to treat said injury or illness as they deem best. I assume responsibility for all medical expenses related to above stated treatment. I further authorize Kennedy Soccer to use the photographic or video images of the registered minor for promotional purposes.
